Smile myelopathy

what are the causes of myelopathy of any type usually involve corticospinal tract?

1direct compression

2vacular compromise as blood vessels to them are end arteries,CST being newest tract having high metabolic demands

3venous congestion(?)

4recent evidences suggest CST may not be sharply somatotropically organised
as we draw it because at present our knowledge comes from experimental studies

5pull of dentate ligament

are there more?

myeloneuropathy
causes of myeloneuropathy

broadly nutritional,infective

1 vit B12 deficiency(SACD)

2 vit E deficiency / friderich's ataxia

3 cu deficiency

4 pyridoxine excess

5 HIV

6 HTLV

7 taboparesis

8 adrenomyeloneuropathy

causes of hung up ankle jerk(waltman's sig):
typically described with hypoyhroidism but can occur with
diabetes
hypothermia
severe oedema
neurosyphilis
neurosarcoidosis
parkinsonism
.................. still more

three primary determinants of eliciting plantar reflex:

1length
2velocity
3pressure

plantar reflex either present or absent:
most of the time equivocal response means one has not put whole hearted efforts to elicit response

Well, these are causes of noncompressive myelopathy. There is compressive myelopathy also.

what i have posted is myeloneuropathy,
and how clinically you diagnose it?
when patient presenting to you with weakness,on thorough examination one found that patient is having absent ankle jerks but bilateral plantars extensor.
one another way of to know whether absent ankle jerk is due to neuropathy or radiculopathy is to check biceps femoris reflex which is absent in radiculopathy and present in neuropathy(length dependent)
